- 60-minute initial adult evaluation (preference for in-person): $375
- 90-minute initial adolescent evaluation (preference for in-person): $425
- 20-minute follow-up (in-person or video): $195
- 40-minute extended follow-up (in-person or video): $275
- 60-minute follow-up/therapy (in-person or video): $375
- No-show or late cancellation (<24 business hours): above appointment charge
- Refill requests outside of scheduled appointments & patient portal messages: $50 per 10 minutes

Contacting Dr. Milmo or Dr. Chatrath does not establish a professional physician-patient relationship. If you are or believe you are experiencing a medical or psychiatric emergency, including suicidal or homicidal thinking, side effects to medication, or any other urgent or time-sensitive matter in which you need an immediate response, do not use this service. Instead call 911, 988, or go to your closest emergency department.
- The practice is not accepting insurance or Medicaid/Medicare at this time
- Payment, in the form of debit/credit cards or HSA/FSA cards, is due at the time of service
- Above rates may be reassessed and adjusted annually at January
- Stating that you did not receive an appointment reminder does not exempt one from the no-show fee
- To maintain privacy and enhance safety, video appointments must be conducted in a location with privacy, access to high speed internet with video/audio capabilities, and not while operating a motor vehicle – ending the appointment if these parameters are not met still come with the expectation that payment for the appointment cannot be refunded
Comments on Fees:
- Upon request, a superbill that details the services and associated fees can be provided to you
- You may be able to submit this documentation to your health insurance company, dependent on your insurance’s out-of-network benefits, for reimbursement
- It is highly encouraged to contact your health insurance company to inquire about these benefits and the process of submitting a claim as it may involve use of a claim form or authorization to seek our services as we are out-of-network
